Oophaga sylvatica (Funkhouser, 1956)
Type locality: "Hacienda Espinosa, elevation about 1,000 ft., 9 km. west of Santo Domingo de los Colorados, Province of Pichincha, north-western Ecuador".
Holotype: CAS -SU 10568.
