Type locality: "quebrada Llamaquizú; a 6 km del pueblo de Oxapampa; 10° 39´ S, 75° 27´ W aprox.; 2,010 m de altitud; ladera occidental del Parque Nacional Yanachaga-Chemillen; provincia de Oxapampa; Pasco, Perú".
Holotype: MHNSM 16542.
